By accepting these terms, you authorize BioQuest, the administrators of this registry,
to store the information that you supply and utilize this information to meet the
objectives of this registry program, including, but not limited to accessing your
patients’ information for data validation and generation of reports. |

Subject management will be decided by you without any imposed intervention in order
to collect data reflecting your actual clinical approach to the management of DVT
and its complications. You agree to upload authentic subject information onto the
registry. The accuracy of this information is your responsibility, as is the information
that can grant access to it, specifically your username and password. |

|
You hereby agree that the Scientific Advisory Committee will be the highest deciding
authority on all issues regarding the use of this registry, including the utilization
of registry data for publications by participating physicians and sponsors. You
will seek the written consent from the scientific advisory committee prior to using
registry information for publications.

|
|
The sponsor will have the right to include trend reports in their publications,
provided the scientific advisory committee has provided written consent for the
same. The sponsor may terminate this registry service at any time with reasonable
notice. You must inform us in writing (not by phone, fax or email) that you no longer
want to store your patient information in this registry.
